  1. Memorandum of Article (M&A)
  2. SSM Certificate
  3. Form 9

  8. Form D, A, B, Superform
  9. All directors’ photocopy of identity card (Front and Back can cross-corner for Payfac ONLY)
  10. Latest bank statements (Minimum 3 months to 2 years of audited account upon request)
  11. Business namecard

FOR ENTERPRISE (PEMILIK TUNGGAL OR PERKONGSIAN)

  1. Full SSM (usually 2-4 pages)
  2. All directors’ photocopy of identity card (Front and Back can cross-corner for Payfac ONLY)
  3. Latest bank statements (Minimum 3 months to 2 years of audited account upon request)
  4. Business namecard

Compulsory Requirements:

  • Merchant must have a shop lot/office/business premise
  • Merchant must have a business license – Lesen Perniagaan or Have Lesen Majlis Perbandaran
  • Merchant must have signboard or / and dimiliki
  • Merchant must have registered company business account in any bank in Malaysia

Merchants Need To Submit:

  • Checklist Info
  • Photo of shop front + signboard
  • Photo of signboard with dimiliki (where available)
  • Photo of shop front + right view
  • Photo of shop front + left view
  • Photo of internal view – counter view (payment counter)
  • Photo of random internal view 5 to 6 pictures min.
  • Physical Merchant Application Form (Sign and stamping page 4 of 7)
  • Physical Merchant Application Form (Sign and stamping page 5 of 7)
  • Physical Merchant Application Form (Sign page 6 of 7)
  • Two copies of RHSB FISERV Payfac Sub-Merchant Processing Agreement (Sign and stamping page 30)
  • Two copies of Revenue Acceptance Merchant Agreement V 2.0 (Sign and stamping page 26)

Return Policy

We do not provide any Return of purchasing after 48 hours.

48 Hour Cancellation Policy

We understand that due to your busy schedule you may have to cancel or change an application with us. In order to respect the time of both our guests and our staff, we simply ask that you notify us of a cancellation at least 48 hours prior to the application. This is so that we may have ample time to process the cancellation should you need to cancel your service. Application that are cancelled more than 48 hours prior will result in a RM150 fee or 50% of the services you are reserved for. ARMIENA Group Sdn. Bhd. reserves the right to refuse service to anyone demonstrating inappropriate behavior to any member of our staff.
